What is Hospice?

Hospice is a philosophy of care with an emphasis on improving quality of life. We provide comprehensive pain and symptom management to allow patients to live comfortably. Hospice care is meant for patients who are terminally ill or have a life-threatening illness. Hospice care is appropriate for patients of all ages.

Who can refer?

Anyone can refer patient to hospice. Our hospice agency will work with you and your physician or the hospice physician to coordinate care.

Where can I receive services?

Hospice patients are cared for in their homes, assisted living facilities, nursing homes and contracted inpatient hospital settings.

Who pays for hospice?

Medicare, Medical / Medicaid Plans, most private health insurance companies and private pay is also available.

Your Hospice Team

Altitude Hospice is dedicated to providing compassionate care & outstanding services. We offer a patient centered approach that treats the physical, emotional, social & spiritual needs of the patient & family. To make that happen, we surround the entire family with an interdisciplinary team that specializes in end of life care.

Medical Director

Overseas pain & symptom management of patient. Consults with attending Physician when needed.

Nurse Care Manager

Provides hands on physical care to ease symptoms and provide comfort for the patient. Coordinates care with the hospice team.

Spiritual Care Coordinator

Helps with spiritual needs as requested by the patient and family. Also offers bereavement support.

Hospice Aid

Provides comfort measures for patient & personal care such as bathing & grooming.

Hospice Volunteer

Provides companionship, runs errands, and in many ways, eases the family’s burden.

Medical Social Worker

Helps bridge emotional gaps, and links the family with community resources. Also assists with end-of-life planning.
